• Certification Number or Social Security Number for the certified nurse.
• Visa or MasterCard only--institutions or facilities with pre-paid accounts can use these, too. (Each verification is $40).

Newly Certified Nurses
Your first verification of certification is free when you are first certified or when you renew your certification. Verifications are not sent automatically to state boards of nursing or employers - you must order each verification. New in 2009: You may use the on-line system to request your free verification, or you may mail your request using this form.
