For Name Changes
	Name changes must be submitted in writing, and should include:
		Full Original Name and Address
		Certification Number or Social Security #
		New Full Legal Name
		Copy of Marriage Certificate or Court Order
		Copy of State Issued Photo ID with either old or new name.
		Verification of your current contact information: home address, email, and phone
