Certification, Renewals, and Verification Requests

ANCC offices are closed  December 25 through January 1. The office reopens January 2 at 9:00 am EST. If you have a certification renewal concern that requires attention, please email certification@ana.org with your concern and the best way to reach you. Staff will respond on December 27 and 30.

For Name Changes
Name changes must be submitted in writing, and should include:

  • Full Original Name and Address
  • Certification Number or Social Security #
  • New Full Legal Name
  • Copy of Marriage Certificate or Court Order
  • Copy of State Issued Photo ID with either old or new name
  • Verification of your current contact information: home address, email, and phone
