The Demographic Data Collection Tool (DDCT) is a web-based portal for managing compilation of required demographic data into a report. This report is submitted to the Magnet Program office with the initial written documentation, redesignation written documentation, and annually by the final day of the anniversary month of the organization's designation. This tool replaces the Demographic Information Form (DIF).
Access information for the system is provided below for current Magnets and those who have completed the application process. If you are going to this site for the first time enter the MPD’s work email address, then select “Forgot Password” link. This will allow you to establish your own password.

- Systems must submit a separate Demographic Data Collection Tool (DDCT) report for each entity and for the system level to account for system-level assets
- Include the most recent annual or fiscal year data
- All information requested is required, unless it is not applicable to the organization
- The Demographic Information Form (DIF) will be accepted through December 31, 2013. Beginning February 1, 2014, organizations must complete the demographic report in the Demographic Data Collection Tool (DDCT).
