		the healthcare organization or hospital code assigned by the American
		Nurses Credentialing Center (ANCC) Pathway to Excellence™ Program
		the ZIP code of your healthcare organization
		the last four digits of your current nursing license
		(as a unique identifier for the ANCC survey administrator)
		to answer every question in the survey

All information is kept strictly confidential and is not accessible to anyone else. Your healthcare organization will only receive a report of the total number of respondents to determine eligibility for designation (requires a 51% response rate or more from a majority of nurses employed) and an overall rating of positive responses. To achieve the Pathway to Excellence designation requires a positive rating of 75% or better, that is, reflecting that nurses consider it a supportive organizational culture.
