The following are required components of the Specialty Nursing Application Packet; it will not be considered complete if any are omitted:

• 1. A completed, signed, and dated ANCC Certification application.
• 2. If the certification examination requires a baccalaureate or higher degree in nursing, then you must submit official transcripts or a copy of your diploma showing conferral of the baccalaureate or higher degree in nursing.
  • For degrees conferred outside of the United States or its territories, please submit a course-by-course evaluation by foreign credential evaluation service. A certificate is not accepted. Click here for more information.
• 3. Applicants for the Psychiatric & Mental Health certification must also submit Form C--Statement of Clinical Supervision and Endorsement of Candidate for Certification as a Psychiatric & Mental Health Nurse.
• 4. A completed Form D--Professional Development Record showing the required contact hours.
• 5. Check or money order payable to ANCC or complete the credit card information section on the application form. Calculate your fees using the fee schedule here.
